First, the page has things like character name, ancesty, background, class, as well as their Class DC

Then Perception and senses

Finally the skills that seem like they are most likely to be in secret rolls\

Updated version (haven't been doing a lot of face to face, so haven't updated lately)

Now has a couple of skills that were missing before, and also is in a 2 per letter sized page version!
