I tried to include everything that might be needed at the table.
Some notes:
I didn't try to condense room 11. Use the module for that one. However, note that none of the negative effects are supposed to be happen until the PC leaves the room. Somebody is likely to hit the worst level. Bwhahahahaha...
I also added a copy of the map for room 6 that includes my best interpretation of how the cells should be numbered. This is the only numbering I could find that met the textual description and didn't create an un-solvable puzzle. Read the example of the first lever-pull over and over until it makes sense. BTW, a medium pawn stuck in the base sideways is perfect for representing the moving walls on a 1" map.
