  You can hand these out to your table and have them fill them out while they fill out the event reporting form. Put an X next to every skill name you want them to fill out the info for.
  
  Or, be a helpful player and print these for yourself and hand a filled out one to your GM -- they will probably appreciate it!

  Can we get an editable version? I have a custom sign-in sheet that my lodge uses, and I'd like to add that and some additional items to the reverse.

  It was made in photoshop, so you should just be able to import the PDF in photoshop and modify to your hearts content. Font is Calibri Regular 14pt and 9pt for the small text

  I added a column for a GM to fill out pre-rolled numbers. I do this at the start of the scenario by dropping a handful of d20s, and writing their numbers down the side of the handout, so when someone wants to make a check, I just check the sheet and use the top number then cross it off.
