  I recreated a similar version of this form with the addition of a "Core Campaign" check box and a set of "Normal" and "Slow" check boxes per player.
  
  The only other additions I was thinking of is a player death check box and an individual player prestige earned field. If anyone thinks those would be useful, let me know.

  I liked the general format of the forms here, but I'm not a big fan of the half-sheet formats. So I created my own. (And then some of the other players/GMs asked if I would post it here.)
  
  It's largely the same as the other one's here, just on one full page. I took off a few things that I didn't feel were necessary (like advancement speed) and added a few things that I wanted to know (like player name and character class/level). I also included an "After Scenario Reference Sheet" (that I found that someone else created) as page 2 for convenience. (Especially if people then print two-sided they can have everything in one place.)
